We have just created a new website; which will be expanding over the next few months. The URL is Crum.us Not only does this website describe our approach and our services, but, it provides webchat assistance from our receptionists. Through the website you can also register to schedule an appointment. Additionally, this new site provides access to our patient portal. It provides extensive patient education tutorials on a range of mental health and neurological conditions.
We also have an app for patients which permits them to log in from their tablets or smart phones to check appointments, send messages, access their account statement, conduct teletherapy sessions and do routine standardized assessments.
All billing questions are handled by Nadine Rutigliano. Please leave your questions for her with our receptionist, and she will call you back.
To insure a smooth billing and payment process we ask that if you are using your insurance, you call to verify your benefits - particularly your copayment and annual deductible, as well as what therapy codes your insurance covers. Our most commonly used codes are 90832, 90834 & 90837 for sessions. For testing: 96130, 96131, 96132, 96133, 96136 & 96137.
We value your privacy. Therefore, if you have issues that you don't want your issurance to have a record of, we recommend paying for your services yourself. Otherwise, if you sign a release of information, we will bill your insurer if we are a provider for your plan. But, in this case, we have no responsibility for the confidentiality of your information when your insurance company requests it once it has left our office and been delivered to the insurer.
Dr. Crum is a participating provider for: BCBS, Children's Medical Services (CMS), CompPsyc, Coicare, Mental Health Network, Multi-Plan (PHCS), New Directions, Tricare, Tricare for Life, Humana Military and Value Options, the VA and the VA Choice Program.
Dr. Crum is also able to provide evaluations and/or treatment through the Office of Vocational Rehabilitation, Social Security Disability and The Brain and Spinal Cord Injury program when they preauthorize such and provide a voucher for same.
Dr. Crum is NOT a provider for: any form of Medicaid, Great West, Golden Rule or Magellan, United Health Care or Amerigroup, HUMANA,Coventry/MHnNet Behavioral Health Life Synch (Humana), If you have one of these insurances you need to pay out of pocket for your services.
We currently have the maximum number of Staywell and AVMED and Beacon patients that we can handle. Prospective patients with these insurances will need to be placed on a waiting list.
Quanitative EEG's, Neurofeedback and Biofeedback, cognitive rehabilitation and light therapy are generally not covered by insurance. If you are in need of one of these services, it is recommended that you see the office manager to make payment arrangements prior to scheduling.

Many people are under the impression that the only way to address physical and mental health problems is through medication or surgery. In fact, stress and lifestyle issues underlie many physical and mental health problems and can be effectively managed nonpharmaceutically. Moreover, as David Wilkerson, author of Anatomy of an Epidemic has documented, long term use of medication can contribute to further physical and mental health difficulties.
Whether you are dealing with mental health issues such as anxiety, depression, or PTSD; struggling with cognitive problems such from traumatic brain injury, dementias or learning disorders or dealing with stress related physical problems such as chronic pain, hypertension, chronic fatigue, insomnia, or gastrointestinal problems; non pharmacological approaches can be effective in helping you manage and reduce symptoms.
Our approach is educational. We partner with you to set your goals and priorities, as well as to help you identify and access resources. We strive to help you master skills that empower you to maintain a healthier life style and promote improved functioning .

If you do not have insurance and are a self-pay patient, sessions are generally one hour in length (CPT Code 90837) and cost $300.00 per hour. The initial session is two hours at a cost of $300.00. Follow up sessions are one hour in length. If this poses a financial hardship please provide your 1040 tax return to be considered for a reduced fee.
Please be advised that certain insurers such as PsycCare and AVMED and New Directions (BCBS) do not cover one hour sessions. If yous wish sessions longer than 45 minutes total, you will need to pay the additional fee out of pocket. This would be $50.00.
Testing is billed at various rates for various types of tests and may or may not be covered by your insurance. If you are seeking an evaluation we will precertify your insurance and notify you in advance of what your personal responsibility will be.
Biofeedback and Neurofeedback sessions are not billable to insurance and are each $150.00 per session.
If the doctor is not a provider for your insurance, we bill your insurer at this rate and your are responsible for the difference between this rate and what your insurer pays.
If the doctor is a provider for your insurance, you are responsible for your deductible, copayment and any denied claims.
Your appointment time is reserved EXCLUSIVELY for you. I do not double book patient. Therefore, it is important that you provide 48 hours notice of a cancellation or reschedule to avoid being responsible for the session fee. Moreover, your progress is dependent upon consistent effort and follow up. For these reasons, to help you remember your appointment, I provide both voice reminders and email reminders.
Appointments are generally 60 minutes in length both begin and end on time. If you have important information or questions you want to cover during your session, consider writing these issues down ahead of time so you can use your session time to your own best advantage.

Answering Service: After hours and weekends, calls are automatically forwarded to our message center and retrieved the next business morning.
Patient Portal: If you are currently a patient, you may always log into your patient portal and send a message to any staff member. Select messages, type the staff member's last name and your message. Then, hit send. This will be responded to the next business day if it is a weekend or holiday.

In most cases, patients without insurance pay full at the time of service and those with insurance pay their copayments at the time of service. But, there are times when extensive evaluations are needed or intense treatment programs which insurance may be unwilling to cover. For those with good credit needing services costing from $2000.00 to $40,000.00, I am now able to offer extended payment options.
